The high rates of water supply shortages in rural communities of Brazil's semi-arid region highlight the need for management models that expand access and ensure greater efficiency in service delivery. This study aimed to evaluate the impacts of the implementation of SISAR in the Sert & atilde;o do Paje & uacute; (PE) mesoregion, considering changes in water access, household adaptations, costs, and self-reported health outcomes. Aligned with the previous studies on SISAR, which employed interviews, document analysis, and the Collective Subject Discourse (CSD), this research applied a similar methodological triangulation, consisting of an online survey of 91 households, analysis of secondary indicators, and qualitative interpretation through CSD. The results indicate expanded access to hygiene facilities, a perceived reduction in waterborne diseases, and improvements in operational sustainability within the communities, although water intermittency persists. It is concluded that SISAR contributes to socioeconomic and sanitary advances, and continuous monitoring and adjustments to local specificities are recommended.
